With Mijn Dossier, you have online access to your personal medical record. Wherever and whenever you want. You can view results, ask your healthcare provider a question, or prepare for your appointment. This gives you more insight into your health. With the Mijn Dossier Amsterdam UMC app, you always have your medical record at hand.

In the Mijn Dossier Amsterdam UMC app, you can:

• manage personal details

• manage and prepare for appointments

• view and send messages to and from your healthcare providers

• view results

• enter medications and allergies

• request repeat prescriptions

• complete questionnaires

• enter data for home monitoring

• upload health and fitness data, including data from Health Connect, if you are enrolled in self-monitoring programs.

Are you already a patient at Amsterdam UMC or do you have a referral from your GP? Then you can create an account for Mijn Dossier yourself via the app. You can easily do this using DigiD.
