MyDHR

MyDHR is an online patient portal giving you access to your health information. If you are a current patient of ACT Health, you can request a MyDHR account. Some of the features and functions available to you through your MyDHR account include:

* Pre-arrival tasks - Complete the pre-arrival tasks from home to save time when you arrive at the clinic.

* Health and Visit Summaries - View, download, or send a copy of specific visit summaries from your health record.

* Questionnaires – Fill in questionnaires prior to your visit.

* Request Health Information - Request a copy of your medical record and download it when it is available.

* Test results – view pathology and imaging test results.

* Current Health Issues - Review your current health issues and update them as needed.

* Advance Care Planning – Review and upload documents, such as advance directives and a living will.

* Organ and tissue donor registration - Registering to be an organ and tissue donor is easy.

* Appointments – View appointments and receive reminders.

* Proxy access – Access your family’s health information through proxy access.

* Upload health and fitness data, including data from Health Connect, when enrolled in self-tracking programs.
